Intake Coordinator

Hanna is seeking an empathetic and compassionate Intake Coordinator to join its fully accredited not-for-profit organization located in Sonoma, California. This opportunity is open to individuals with strong interpersonal skills and a passion for serving the diverse needs of others.

About The Opportunity
As a member of the Hanna team, the Intake Coordinator will provide empathetic and compassionate support and guidance to individuals and families seeking mental healthcare services within the local community.

The Intake Coordinator will work collaboratively with members of the mental health team to address and monitor the diverse needs of clients while aiding with obtaining mental health services and support.

In addition, the Intake Coordinator will:

  • Maintain a smooth and efficient admissions process
  • Obtains prior authorizations from MCOs on behalf of patients
  • Audit patient charts to ensure information is accurate and complete
  • Assist clients with offsite referrals and recommendations as required
  • Prepare written reports of assessment results and care requirements
  • Promptly respond to inbound phone inquiries related to mental health services
  • Perform patient assessments to determine appropriate treatment requirements
  • Document clinical assessments and oversee the facilitation of care on behalf of patients

Requirements:

  • Valid California driver’s license
  • Must clear background check for a Community Care Licensed facility, including physical,
    tb and drug test, COVID19 Vaccination and booster or medical/religious exemption.
  • Must successfully complete: Mandated Child Abuse Reporting, Ethical Standards, and
    Crossing Boundaries of Employee Conduct.
  • Must have valid Driver’s License and ability to be insured by Company policy.
  • COVID-19 vaccination and booster required or verified medical/religious exemption
